COORDINATORS

Our patients are always number one. That is why we offer on-site Care Coordinators to help you navigate your healthcare needs. After an assessment by your physician, or a hospitalization, a Care Coordinator can be assigned to create a treatment plan with your physician to monitor any health needs. These Care Coordinators follow our patients along the continuum of an acute or chronic health problem, even assisting in scheduling follow-ups from hospitalizations or Emergency Room visits. Contact us today to find out more information.

A FOCUS ON  WELLNESS

Our team is here to help you on your wellness journey, and strongly recommend you have a wellness physical once a year. An annual wellness examination provides our physicians with the information necessary to ensure you maintain a healthy lifestyle and is also key to early diagnosis and treatment of potentially serious diseases. 

 

Many insurance policies now cover most, if not all, the cost of a wellness examination. We also provide Medicare Annual Wellness visits. These visits allow us to provide you the best preventative care at the right time.
